What makes OneSource one of the best home health options?

OneSource Home Health services are specifically structured to provide the highest quality of in-home healthcare solutions and services to you and your loved ones. Individual situations and needs are always unique. That’s why our homecare experts work in partnership with your physician, sit down with the patient and family, and create a plan of care which offers the best solution to help the patient recover and regain the highest level of independence possible.

We have highly-trained specialists in all areas of care to ensure that you and your loved ones receive the best care possible, whether you have long or short-term needs.

  • Wound care
  • IV infusions
  • Cardiopulmonary assessment
  • Diabetic teaching
  • Post-surgical care
  • Chronic disease management
  • Medication reconciliation and management
  • Gastrostomy tube care
  • Injections
  • Blood draws
  • In-dwelling catheter care and management
  • And so much more

Occupational Therapy

Our highly-trained occupational therapy practitioners assess limitations and challenges and create a unique plan for each individual to enhance lifestyle and living situations. Occupational therapy is incorporated into everyday life to improve health and well being, as well as to remove or reduce restrictions. Whether the aim is to recover, develop, or improve ability, each plan is personalized and strategically developed to ensure the best possible results. This therapy includes upper body mechanics training, adaptive device training, as well as training focused on daily living activities.

Speech Therapy

Beginning with an assessment, speech therapy at OneSource is structured around an individualized treatment plan designed to help with speech, language, and cognitive communication. Using carefully designed and proven exercises, a speech therapist will work with patients to enhance their ability to communicate and can even help with drinking, eating, and swallowing, as well as articulation, word recognition, and dysphagia.

Certified Nurse Assistant

Our team of certified nursing assistants supervises patients with daily living while providing personalized care and attention. A nurse assistant can help patients bathe, eat, and dress, while carrying out important health checks and monitoring a patient’s general well-being—all in the comfort of the patient’s home environment. These trained individuals offer a level of care and compassion that is essential to the role, and have an important impact on whether or not a patient can remain at home throughout treatment.

Social Work for Planning and Counseling

Social work centers around human development and well-being and can be instrumental in recovery or improvement of an individual’s situation. OneSource professionals provide psycho-social assessment and contacts for community resources and programs that can help rehabilitation, healing, and coping.

Dietician Counseling

Our nurses focus on dietary management through thoughtful consideration of each patient's conditions. We discuss and educate around the foods that should be included and avoided according to each patient’s needs. Oral nutritional support includes advice on dietary fortification, eating patterns, meal planning, and nutritional education. Our qualified nurses assess, diagnose, and treat patients based on the latest scientific research around food, disease, and healing.

Physical Therapy

Designed to ease pain, our physical therapy program increases mobility and function, as well as improves a patient's overall quality of life. Our specially trained therapists focus on lower body mechanics training, weight transfer, gaits, balance training, increasing ranges of motion, general strengthening, and bed mobility training.

Increased Comfort through Home Health Care

There’s no place like home, and we here at OneSource want to make it as easy as possible for patients to receive treatment in the comfort of their own surroundings. It’s no surprise that enabling independence, enhancing comfort, and avoiding inconvenience has a huge impact on mental wellbeing. Traveling to and from appointments can be tiring, hospital stays can be distressing, and feeling uprooted from our home can cause distress, unease, and anxiety. Through the help of our occupational therapists, and the rest of our team, we make staying home not only possible, but safe and enjoyable too.
